To participate in sports for the current school year, all athletes (grades 5-8) must complete the following before they can practice or participate:
- Current Physical (No less than every other year with April 1st as the earliest date) Physical copy must be returned to the Middle School Office.
- Available through Bellin or Prevea for a fee.
The following forms must be completed online in the Powerschool registration portal. Parent access with username/password was provided at registration for school. You may access this sports portion even if you have already submitted before. No paper forms will be received at the office.
- Emergency Information, including proof of medical insurance and insurance numbers.
- Concussion agreement - Parent and Athlete Agreement Please read the Fact Sheet for Parents and Fact Sheet for Athletes before signing electronically.
- Mandatory viewing (by athlete and a parent/legal guardian) of the Code and Rules Video, one time while a student at the Intermediate or Middle Schools.
