- Proof of Residency, must be one of the following:
- Mortgage Statement
- Lease Agreement
- Wisconsin Public Service Bill
- Birth Certificate for each student and Legal Name Change Court Order if applicable
- Immunization record for each student or signed waiver
- Health Insurance Card for students registering in grades 5-12
- Two emergency contact names and phone numbers of people who can be contacted in case of an emergency if parents can not be reached.
- Please plan at least 30 minutes for the meeting
